Parents’ Night Out Registration

Waiver and Release of Liability: In consideration of being allowed to participate in the Parent's Night Out event at Team Casarez Jiu Jitsu, I, the undersigned, acknowledge and agree to the following: Assumption of Risk: I understand that participation in martial arts and related activities involves inherent risks, including but not limited to physical contact, falls, and other potential hazards. I voluntarily assume all risks associated with my child's participation. Medical Consent: I authorize Team Casarez Jiu Jitsu staff to seek emergency medical treatment for my child if necessary. I understand that I will be responsible for any medical expenses incurred. Release of Liability: I, on behalf of myself, my child, and our heirs, executors, and assigns, hereby release, waive, discharge, and hold harmless Team Casarez Jiu Jitsu, its owners, instructors, staff, and volunteers from any and all claims, liabilities, demands, or causes of action that may arise from my child's participation in the event, including but not limited to personal injury, property damage, or loss. Behavioral Expectations: I understand that my child is expected to follow all rules and guidelines set forth by Team Casarez Jiu Jitsu staff. If my child fails to comply, I may be contacted to pick them up early. Photograph and Video Release: I grant permission for Team Casarez Jiu Jitsu to take photographs and videos of my child during the event for promotional and educational purposes.
